HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
We are required by applicable federal and state law to maintain the privacy of your protected health information (PHI). We are also required to give you this Notice about our privacy practices, our legal duties, and your rights concerning your health information. We reserve the right to change our privacy practices and the terms of this Notice at any time, provided such changes are permitted by applicable law. If we change our Notice, we will make the new Notice available upon request.
How We Use and Disclose Your Health Information
We use and disclose health information about you for the following purposes. We do not need your written authorization for these uses:
- Treatment We may use or disclose your health information to a physician or other healthcare provider providing treatment to you. For example, your endodontist may share information with your referring dentist to coordinate your care.
- Payment We may use and disclose your health information to obtain payment for services we provide. For example, we may disclose information to your dental insurance carrier to process a claim for treatment you received.
- Healthcare Operations We may use and disclose your health information for our own healthcare operations, including quality assessment, training, accreditation, and business management. This allows us to run the practice and provide quality care to all our patients.
- Required by Law We may use or disclose your health information when we are required to do so by applicable federal or state law, including in response to court orders, public health activities, or law enforcement requests as permitted by HIPAA.
- Family and Friends We may disclose your health information to a family member, friend, or other person to the extent necessary to help with your healthcare or payment for your healthcare — but only if you agree that we may do so, or in emergency circumstances where we use professional judgment.
- Persons Involved in Your Care We may use or disclose health information to notify a family member, personal representative, or another person responsible for your care of your location, general condition, or death — if you are present and have the opportunity to agree, or if an emergency requires us to act using professional judgment.
- Marketing We will not use your health information for marketing communications without your written authorization. You may revoke any such authorization at any time in writing.
Your Rights Regarding Your Health Information
You have the following rights with respect to your protected health information. To exercise any of these rights, please submit a written request to our office.
- Right to Inspect and Copy You have the right to inspect and obtain a copy of the health information we use to make decisions about your care, including your medical records. We may charge a reasonable fee for providing copies.
- Right to Request Amendment If you believe health information we have about you is incorrect or incomplete, you may ask us to amend it. We may deny your request if the information was not created by us or if we determine the information is accurate and complete.
- Right to an Accounting of Disclosures You have the right to request a list of disclosures we have made of your health information, other than for treatment, payment, healthcare operations, or disclosures you authorized.
- Right to Request Restrictions You have the right to request restrictions on how we use or disclose your health information for treatment, payment, or healthcare operations. We are not required to agree to your request, but if we do agree, we are bound by our agreement.
- Right to Request Confidential Communications You have the right to request that we communicate with you about your health information by alternative means or at an alternative location — for example, requesting that we contact you only at a specific phone number.
- Right to File a Complaint If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Uses Requiring Your Written Authorization
Other uses and disclosures of your health information not covered by this Notice, or not permitted by applicable law, will be made only with your written authorization. You may revoke any written authorization at any time by submitting a written revocation to our office. Revocation does not affect disclosures already made in reliance on your authorization.

Website Privacy Policy
Last updated October 20, 2026. This section explains how we collect and handle data through this website (endospecialists.com). This site does not have online booking, patient intake forms, or any mechanism for submitting protected health information. Any appointment requests result in a callback from our staff.
Information We Collect Through This Website
We do not collect your name, email address, or other contact details simply because you visit this website. The services below receive limited technical information, such as your IP address, browser type, and the pages you view, when they are used.
- Google Analytics 4 (GA4) — only with your consent Google Analytics helps us understand which pages are visited, how long visitors stay, and how they arrived. It does not load, and sets no cookies, until you click Accept in the cookie banner. When enabled, it collects device type, browser, approximate region, and pages viewed. Google signals and ad personalization are turned off. You can change your choice at any time with "Cookie settings" at the bottom of every page.
- Cloudflare — hosting and visit counts This website is hosted and protected by Cloudflare, which processes standard request information (such as IP address and browser) to deliver the site and block abuse. We also use Cloudflare Web Analytics to count visits. It does not use cookies and does not track you across other websites.
- Google Search Console We use Google Search Console to monitor how this website appears in Google Search results — including which search terms lead visitors here and how pages rank. Search Console does not collect information about individual users visiting this site.
- Google Maps Our location pages include maps embedded from Google Maps. When a map loads, Google receives your IP address and browser information and may set its own cookies under Google's privacy policy.
- Vimeo The GentleWave page includes a video hosted on Vimeo. The player is loaded with Vimeo's "do not track" setting, which stops Vimeo from tracking session data and setting tracking cookies.
- OpenFreeMap The map on our Patients page loads map tiles from OpenFreeMap, which receives your IP address and browser information to deliver them.
- Cookies and browser storage Google Analytics cookies are set only after you accept. We store your cookie choice, and any accessibility preferences you set, in your browser's local storage so the site remembers them. Embedded maps may set cookies of their own. You can also control or delete cookies through your browser settings.
- Global Privacy Control and Do Not Track If your browser sends a Global Privacy Control (GPC) signal, we treat it as your choice to opt out: Google Analytics stays off and the cookie banner is not shown. There is no agreed standard for the older "Do Not Track" browser signal, so we do not respond to it separately — but Google Analytics stays off unless you accept it.
California Privacy Rights (CCPA / CPRA)
If you are a California resident, the California Consumer Privacy Act (CCPA) and the California Privacy Rights Act (CPRA) give you specific rights regarding your personal information collected through this website.
What we collect: If you accept analytics, this website collects usage data through Google Analytics 4 (GA4) — including pages visited, device type, browser, and general geographic region. This data is processed by Google under its Analytics Terms of Service. We do not sell your personal information or share it for targeted advertising.
- Right to Know You have the right to request information about the categories and specific pieces of personal information we have collected about you through this website.
- Right to Delete You have the right to request deletion of personal information we have collected from you, subject to certain exceptions.
- Right to Opt Out Google Analytics is off unless you accept it. You can withdraw at any time with "Cookie settings" at the bottom of every page, and we honor the Global Privacy Control signal as an opt-out. You may also disable cookies through your browser settings.
- Right to Non-Discrimination We will not discriminate against you for exercising any of your CCPA or CPRA rights.
To submit a data request or exercise any of the above rights, contact us at (510) 451-6828 or visit any of our four East Bay locations.
